Safety violations on job sites are rarely committed out of ignorance. In most investigations, the person involved could describe the correct procedure accurately. They simply were not following it, and had not been for some time.
The violations that recur most
- Fall protection worn but not connected, or connected to an unsuitable anchor
- Scaffolds altered by non-scaffolders, ties removed to make access easier
- Permits signed without the isolation being verified at the point of isolation
- Confined space entry without gas testing, or with a detector that was never bump-tested
- Guards removed from rotating machinery to speed up a repetitive task
- Excavations entered without shoring because 'it is only a metre and a half'
- Lifting over occupied areas because the exclusion zone would delay other trades
- Housekeeping ignored until an audit is announced
Why they happen
Almost every one of these is faster than the compliant alternative, and almost every one has been done successfully hundreds of times before. That history is the problem: each successful violation lowers the perceived risk of the next. Safety researchers call this normalisation of deviance, and it is how experienced, careful people end up in serious incidents.
The impact
- Injuries and fatalities, with consequences that reach far past the individual
- Project shutdowns and regulatory action from the relevant authorities
- Loss of contractor prequalification with major clients — often the costliest outcome
- Insurance and claim consequences that outlast the project
- Erosion of workforce trust once it is clear the rules are optional
What actually reduces them
Not more rules. Removing the conflict between the rule and the schedule, making the compliant method physically easier, and having supervision that corrects every time rather than occasionally. Consistency matters more than severity.
